Guys - if I simply call 911 (not using emergency sos side button), does the phone still notify my emergency contacts

No, in the several times I've called 911 - it does not notify emergency contacts. Emergency contacts have only been notified when using the Emergency SOS side button (which mother in law has done) as well as call 911.
 
Short answer is No. It will not notify your emergency contacts if you dial 911 by yourself. A simple reason is that you could be calling for someone else. You don’t want your family to get notified just because you being a good citizen and calling for a stranger in need, right?

For using the Emergency SOS manually, you can choose whether to notify your contacts or not, as stated in the FAQ from Apple.
With Emergency SOS, you can quickly and easily call for help and alert your emergency contacts.

Once the call ends, you can choose to notify your emergency contacts with a message that says you have called emergency services. This message also includes your current location.

However, if you are referring to the Emergency SOS automatically triggered by fall detection, it WILL automatically send a message to your contacts.
After the call ends, your watch sends a message to your emergency contacts with your location letting them know that your watch detected a hard fall and dialed emergency services. Your watch gets your emergency contacts from your Medical ID.
