Is it possible to change the EQ of the Apple Watch series 4 when listening to offline music and connected via Bluetooth headphones?

I noticed a huge difference as I usually use the EQ function on my iPhone and it is a night and day difference when listening to music compared to the Watch.

Not unless you apply an SQ setting to a certain album in iTunes itself. Not sure if this is still a thing, but you used to be able to do that.
 
I wish there was. Especially when I’m listening to music in my car from my watch.

The Apple Watch should have more music functionality than the iPod Nano, not less.
