Hello, My iMac keeps rebooting and giving me this error message below, I booted in to Diag mode and did an extended test and everything came back OK, So I know there a lot more smarter people on here than me so maybe one of you can tell me what this means.

this is advice for any mac problem but yours is a disk error, please go to message 4

start with the easy stuff.
erase prm
https://support.apple.com/kb/PH14222?locale=en_US
erase smc
https://support.apple.com/en-la/HT201295

disk utility won't tell you the "smart" condition anymore and apple hardware test will not tell you if your hard disk is still good. all apple hardware test does is check to make sure the hard disk is installed. so therefor you need to check the "smart condition" is normal. and the disk is good

this means going into about this mac (if you can ) , system report, storage. it should say s.m.a.r.t. status verified if it says anything else , like failed or failing, it means your hard disk is totally shot and you need to buy a new one, period end of story.

smart status also applies to SSDs.

boot hard disk into recovery mode and run disk utility to repair permissions and repair the disk.

erase hard disk and reinstall os x. and restore from time machine.
if you have a SSD do a simple erase.
if you have a mechanical hard disk do a complete 1 pass to make sure you don't have any new bad sectors.

if none of this works, its time to take it to a genius. the genius is basically going to do everything i said.
so if you can show him how your computer passed apple hardware test, then his tests are not really going to do anything that AHT didn't already do. except give you a GUI

when geniuses are stumped they will just tell you that you have to turn off your computer off once in a while. you tell them this guy googled this and he said my computer was only on for 7 minutes and i want to talk to a different genius.

i google this and hfs_UNswap_BTNode specifically means you have a hard disk problem.

AHT does NOT TEST hard disks.
verify disk is good by checking smart sensor, repairing permissions, repairing disks, and if all else fails erasing and reinstalling.

if smart status shows the drive is bad, have it replaced immediately.

like i said , system report is the fastest way to check the smart status.

here is a screen shot

ran the repair permissions and a lot of permissions were repaired, did a verify disk came back good, did a repair disk came back good, the only issue that came back was

Verifying permissions for “Macintosh HD”Warning: SUID file “System/Library/CoreServices/RemoteManagement/ARDAgent.app/Contents/MacOS/ARDAgent” has been modified and will not be repaired.

Its fix now,

I found that I could replicate the issue by trying to do a manual back up with time machine

I talked with Apple support, we did a OS reinstall, they told me they would call back ion an hour to see if ai still had an issue, and if I was they wanted to format my drive and reinstall from scratch, since I had a back up, well it was still having an issue so while I was waiting for them to call back, I wanted to make sure I had a current backup, every time I tired to force a backup now, it would reboot, it seems the hard dive went carupted on the apple airport extreme. I erased the hard drive and remounted it in time machine and its all working now.

thanks for all your help.
