Every iPhone i have owned for years I end up with this problem. The automated recording says please press a number and nothing happens on the other end as it doesn't hear or recognize the beeps. This stops if I restart my phone but in a call I cannot get back to the original call. Does this happen to anyone else.
 
Sounds like a carrier issue, or an issue with the IVR you're interacting with to me. I've never run into this.
