All of the sudden when I go into the Apple next to the finder at the top of the screen there's two of everyting in the drop down menu.. 2 Force Quits, 2 restarts, 2 log-outs, etc,. etc.,

Also there's always a clear box with a thick black outline floating around- it never goes away.. Right now it's on the back arrow in Safari..

Anyone ever heard of this before?

This is on a 1.9 GHZ, G5 iMac running the latest version of Tiger with 1.5GB Ram, and plentyof HD space left..

turn off voiceover in system preferences-> universal access.
 
If that does not solve the problem, open Applications->Utilities, type
Code:
/System/Library/Frameworks/ApplicationServices.framework/Frameworks/LaunchServices.framework/Support/lsregister -kill -r -domain local -domain system -domain user
and press return, then reboot.

The problem should be fixed.

This command tells the system to rebuild its index of applications.
