I am trying to finish up a form that I created for my company. There are several check boxes on the form that are in place on the electronic version, but when I print it out, the boxes (whether checked or not) do not print. In the properties, the box for "print icon" is selected. Here is a photo to illustrate what I mean. The box is at the arrow.

Also-- the boxes don't seem to want to stay put. Any help is greatly appreciated!

I had the same problem, for which I never did find a solution. If you've got Numbers, open up the spreadsheet in Numbers—happily, Numbers is actually able to print checkboxes.

I think the fact that the checkbox toolbar looks like it has since, oh, Word 95 suggests that checkboxes aren't much of a priority for the Excel developers...

Known issue, we're working on it, but I don't have an ETA for a fix. Sadly, I'm not aware of any workaround for it now. :(
