I'm currently on T-mobile but I have an Iphone that is within its warranty, If I bring it into apple to try to exchange it for a refurb or new one within the warranty period if something happens to it, will I need to have an activated att sim or can I go in there simless or with with an unactivated att sim?

no just restore it thru the itunes website...remove your tmobile card and thats it...they do not ask for a phone number

No, you do not have an iPhone that's within warranty because you voided your warranty when you unlocked it.
