I write this on a MacBook Pro 17" (early 2011) with macOS Mojave (thanks to dosdude's patch). My USB 3.0 ExpressCard (34 mm) works fine until I (mostly) inadvertently eject it (it is all to easily ejected). After ejection, the icon in the menu bar vanishes, and reinserting the card does not trigger a reload of the kext or whatever mechanism is needed to reactivate it. The card is a no-name card from China (nano tech).
