External monitor hot plug problems

Is anyone else having problems with external monitor hot plugging? My monitor often just stays blank and significant input lag can be seen on the laptop itself. A restart usually fixes this issue, but a similar problem occurs on the MacBook Pro side when the cable is unplugged.

I am using a mini-displayport to DisplayPort adapter to connect to my monitor (via DisplayPort cable), and I am using a 2012 MacBook Pro (non-retina model).
