Fastcontacts - How to add groupings?

I bought FastContacts which is fairly highly rated but I can't figure out how to make groupings. It tells me to use the address book but FC on the iphone doesn't show my groups and the grouping show in FC (All Contacts, Last Name) are not groups in my address book.

I check the support page, no help.

How is this thing supposed to work?
