Faulty Audio Output

I have a unibody MPB and I'm having a lot of issues outputting audio. The sound quality is fine, but periodically it will stop outputting and just start playing through the internal speakers when the cable is plugged in. The cable is NOT the problem. I am an audio technician and have troubleshooted this to the best of my knowledge. It mainly happens when I pause itunes then play again. It is also totally random. Sometimes it happens, sometimes it does not. Has anyone else experienced this?
