FedEx signature form

I needed this form also. I have more than 1 iPhone being delivered from FedEx tomorrow so it seems a single form would suffice for multiple packages at the same time, right? I know the UPS form has a field for tracking number, which this FedEx version doesn't.
Also, I'm seeing FedEx Employee Name and Employee ID fields above the "For FedEx Internal Use Only" area. Anyone know if that info is required for this form to be valid?

I just called FedEx and they stated that packages showing "Indirect Signature Required" means that the release form will not work. So YMMV with this release form.
I called fedex a few days ago and they said it was able to be done with simply a note with your signature, so who knows.... I’m gonna call again and get a confirmed answer.
It’s worked for 5 years straight. Even says so on their website. ‘Direct’ signature required is the one that requires someone be there to sign.
So for those that have used this form, what did you put for the FedEx Employee Name and Employee ID fields?
Yes!! I used the one right above your comment and the guy left it with the tracking saying “release form collected.” Even with the fedex people saying they couldn’t do it he still did, unless this guy just happened to be cool bout it.
