Just noticed this today. Attached png. One of my .mp4 files just appeared on the top right of the window. Doesn't matter which folder I open, that same file remains. It was on my desktop, so I decided to trash and delete it to see if it would disappear. But all it did was give me the "?", instead of the QT icon. I've never seen this happen before. The name of the file is gone now, but the "?" is still there. How does this happen, and how do I get rid of it?
