Hey there,
I recently had this issue on my MacBook Pro 15 Inch Retina 2016 model.

I've attached a screenshot of the error I get when I run disk utility.
Does anyone know how to overcome this? I've done a reformat.
What's weird is that I can reboot my Mac and have a successful first aid diagnosis with no corruption and then immediately get this when I try to run it again.

1- Failing disk -
2- possible 3d party app (turn them all off), remove form starting at booth, just make sure none is running at all.
If you have stuff like Clean my Mac, and other garbage, do not use it.

As it suggests, run the First Aid from Recovery mode.
 
There are some repairs that cannot be done on a running system.
Run from Recovery. You might have to mount first in the same DiskUtility under Recovery, if you are using Filevault.

If you have a second boot partition that is High Sierra or Mojave, you can also boot into that and repair.
 
I have this problem too. The first run after a reboot gives no errors. The second run fails with the same error. A complete wipe and reinstall of MacOS didn't fix it. Must be a bug or something.
