Was wondering if anyone knew of a way to create email-able forms in Apple Mail.

Currently, I use faxes for clients to sign off on jobs and they fax them back to me.

But I would love to be able to create a digital form, that they could type in their name, click an approve or disapprove dialog, and maybe date it. (or something along those lines)

Would be a big time save. Looked all over for 3rd party apps or plug-ins, but came up empty.

It sounds like you need to use digitally signable PDFs.
