I didn't make this app, but I found it while searching SlickDeals. Because of that, I don't know how long it will stay free, or if they will eventually charge for the app.

I've tried messing around with it (not in an accident, thank god!) and I have to say, the features are pretty neat! You pre-fill your info such as driver's license, tag number, address in the app and when you need to use it it asks for the other driver's info, witnesses, GPS map of where it happened, pictures of the damage and a MS-Paint-like drawing program to draw a quick map. You can finalize everything by having it automatically email all that data to you and.or the other driver if needed.
I've spotted a bug here or there (misspellings, option that doesn't work) but overall a solid application. I'd probably pay a dollar or two to buy it, but it's free! Now lets hope none of us have to actually use it...
