If I want to reinstall OSX, do I format the entire drive, including the recovery partition, or only the OS partition?
 
No, not normally, unless that's how you want to proceed.

Reinstalling OS X doesn't require you to format anything, unless it's something that you feel is necessary because of your symptoms.
 
I have always just upgraded since Lion. I want to start fresh with Mavericks.

How are you installing? If you made yourself a USB key installer, then it does not matter. Just erase Macintosh HD then install Mavs. That will install the OS on the Macintosh HD partition and it will replace the previous 650MB recovery partition with a Mavericks version of recovery.
 
I got it, just went into disk utility and formatted, did the download of Mavericks and installed.
