As a final shot in the dark on a G5 I've been trying to save I decided to check for a firmware upgrade.

Based on the serial # and model # everymac.com reports it as being "Original/Late 2003".

Visited Apple.com and searched for "Power_Mac_G5__Firmware_Update", it returned several. Based on the information returned from everymac.com I ruled out all the updates listed as "Early 2004" or "Late "2004". Two remained, "Power Mac G5 Firmware Update 5.1.4" and "Power Mac G5 Uniprocessor 5.1.5 f2 Firmware Update".

My 'good' G5 (same family / model) reports a ROM version 5.1.5f0. The 'troubled' G5 is a Dual processor, not Uni. So I'm not certain which, if either, to try.

Besides giving up, any insight or suggestions?
