Does your acoustic really have that wide sound hole, or is the photo stretched (for the icon, I went and assumed it was really an unusual shape)?

It does, in fact, have the odd sound-hole. It's part of the reason I love the look
It's just Photoshop and a plugin to save as ICNS. The shadows are just generic black ovals with severe gaussian blur and the opacity turned down, something a little more careful would probably be wanted once Leopard and bigger icons start appearing.
