For example, when the standard save dialogue appears:

Don't save -- Cancel -- Replace

Half of the available options can I access only with the mouse, it seems.

I'm coming from MS Windows and are used to have the TAB key to select an option, then press Return to execute the selected option.

Or I press ALT, then go through any menu using the arrow keys, select something, and press Return.

Am I lost in Mac OSX without a mouse? I don't think so. What's the trick?

Sometimes it's nicer for the workflow if I can keep both hands on the keyboard.

In System Preferences, go to Mouse & Keyboard>Keyboard Shortcuts and turn on full keyboard access.

This will let you tab through buttons & fields.
 
Also go to system preferences> Expose

if the previous instructions still don't open it up fully.
