Feel free to chime in if you have a perfect replacement or not.

Yellow screens, interlacing issues, etc...

I had mine replaced last week after an update to 4.1 seemed to wreck my original iPhone4.

The genius bar replaced it straight away. The new one is fantastic in comparison (battery life and call quality).
 
My first one had Flash storage issues 2-3 days after purcahse, and would randomly do the black screen of death, completely turning off. After a few DFU restores, one failing with the Apple tech, I got an exchange.

The exchanged one so far has been perfect. So far, so good.
 
I need to make an appointment. Even after upgrading, I am still having proximity sensor issues.
 
Had mine replaced today and so far it's much better than my 1st.

The screen does have a warmer look to it, it's much easier on the eyes than my last one that had more of a blueish look. Battery life is 10x better and no more proximity issues. *crosses fingers*
 
Had to get three 3G replacements a few months ago (following a water based user error). First one had serious antenna problems, then the replacement replacement had a faulty mute switch. Third replacement is ok so far as know (is on 'loan')
 
I got my replacement Friday. I was having dropped calls. I had hoped it was the phone. Unfortunately, it seems to be AT&T. Now I have a refurb in place of my new phone. Still love this phone though.

Funny, my 4 year old son likes the iPhone better than an iPad. I guess he's getting an iPod Touch for Christmas!:D
 
New phone works perfect except the signal is bugging me sometimes.

When i restart the iPhone it says No Service where as my other one would grab a signal right away. I have 3-5 bars of Edge and 3 bars of 3G. I live 12 miles from a cell tower on flat land. Also installed a new simcard from AT&T and reset network settings.
 
Got a replacement last week (Week 35) - however, it was 5 weeks older than the original (Week 40), which clearly means it wasn't "new" as the Genius stated.

Everything runs fine, but I have spotted a dead pixel on the right hand side of the screen.

Do you guy's reckon it's worth replacing again? it's not visible unless you're REALLY looking for it - but I just hate the fact that I've spend a lot of money on this phone (bought sim-free), only to find out that it's a refurbished model and has a dead pixel. Sort of takes the gloss of it, I feel a little cheated... if you know what I mean.
