So in the home screen, at the bottom right, there is a row of icons, like the camera, wi-fi or question mark icons.

Next to the question mark, there's a monitor, and for as long as I've used Goodreader, it's always been disabled. What is that for? Is it for displaying files on an external display?

That's the "AirPlay" icon, so yes, it's for displaying files on an external display (or sending audio to external speakers). If you have AirPlay enabled, and are on a network with AirPlay devices, the icon should become active.
