Greyed out song on iPhone won't go away

I have a single greyed out song in my Music app on my iPhone that won't go away. I've tried deleting all music on the iPhone (General->Usage->Music-> Delete All) and it's still there. I've tried re-syncing my entire library and it's still there. I cannot manually delete it by swiping... how do I get rid of it?
