Best thing, is to decide if its worth keeping or not. If not, delete it. If it is, but isn't use much, back it up on a disk, iDisk, CD, ect and then delete the item from the internal hard drive. Store movies/TV shows you don't watch much, on an external drive to free lots of space. You simply have to plug the drive in, click on the file, and its re set back in iTunes.

ok...... but i don't have an external hard drive
 
These days, they are real cheap. As low as $69 for 80 gb.

A quick and cheap alternative, if you can burn DVD's, is open the iTunes folder, not the programs, and drag and drop the TV or movie files to the DVD and create a backup of them. They won't play in a player as they would be data backup, but its possible to remove lots of files that way.
