After being on charge yesterday my wife's Apple Watch Series 4, 4 years old has become unresponsive. We have tried all suggestions on how to reboot, recover etc but to no avail. Could it need a new battery?.

That is the mode where it shows the clock without booting WatchOS. It suggests that the battery is empty. Place it on the charger for an hour and then take it off, hold the power button for 10 full seconds, and see if it starts up. If not, hold the power button for 30 full seconds, wait and see if it boots and if not, hold it again for another 5 seconds and then see if it boots.

If that does not work you'll need to bring it into Apple and they can run diagnostics in the store to check if it needs a new battery or if it's otherwise defective. They will tell you what it costs up front and you won't be charged any more than that.
 
We have used the charger that came with the watch and all seems OK!, with no need to reboot, recover. We are of the opinion that the Belkin 2 in 1 multi charger the wife has been using is the culprit?. To be honest I have a Belkin 3 in 1 that seems to malfunction from time to time. Perhaps they a have a design flaw?.

The Apple charging puck is incompatible with a lot of 3rd party chargers. Unless you plug it into a Mac or Apple charger, it might actually not charge at all. It is very annoying as I've had it on the charging puck overnight to find out the next morning it didn't charge at all.

Maybe a flaw in that I wonder if the Watch is not seated solidly in vertical? Magnet is a bit too weak, especially if band is on the heavy side? But judging from the image, looks like a normal type of band.

Nightstand mode on? Tap the Watch when attached to see if you get the ”on charger face” to see if seated properly.

Have not run into this, but then again, I’m only using Anker chargers or an old Apple 5w cube for charging with the Apple puck. And also have a 3rd party pick bedside into an Anker and have not had issues.
 
One of the things it does not like charging from is USB hubs. The AW charger plugged into the hub does not work at all... until I plug the Mac into the hub at which point it suddenly starts charging. It makes no sense because the hub is connected to power separately and does charge other Apple devices like my iPad just fine without having to plug in the Mac as well.
