I have a 40GB 4th Gen iPod (B/W screen), I started it up in diagnostic mode (Select + Rewind while booting up) and ran the HDD SCAN test. Here's what I got for results:

Code:
HDD SCAN

112
RD FAIL
SCAN NG

Any idea what that means? Much thanks!

I think RD FAIL means it can't read the disk, but could well be wrong - I usually am. Can you reformat it with Disk Utility? Is it being recognised by computers?

Got the same thing myself while trying to verify possible bad sectors on the drive. I was thinking 112 might at first be the number of bad sectors, but as you got the same thing I'm guessing it's an error code instead.

Either way, the drive can't be read in one or more places due to bad sectors I reckon. Whether or not the scan logs the sectors is another matter entirely. You should try to erase the drive and choose the option to write zeros - this should identify and bypass bad sectors.
