In preparation for delivery of the nanos I ordered I started trying to build a multibook playlist. (Until now I've just ben using Shuffles for music lists.)

All files are .mp3.
Each book is in its own folder. Each such folder contains a subfolder named "Ch## ChapTitle" and each chapter folder contains tracks named "Ch## ChapTitle" - trackNum."

But when I drag the bookfile into iTunes it eliminates the folders and just stacks the files.

So when I drag a second book to the playlist window the chapters re-stack alphabetically, putting chapter one from one book right after chapter one of the other.

How can I build a playlist with the obvious hierarchical structure of booktitle/chaptitle/chaptrack?

You could create a folder for the bookname, playlists in that folder for the Chapter title, and in that playlist put in all the chapter tracks.
 
Although I'd suggest you just merge those chapter tracks together to make 1 file per chapter. Audiobooks resume where they left off so you don't have to find your way through every time.
