Help restoring iphone 4..

So I drank too much and cracked my iPhone 4 screen. Phone still works great but it looks like ****. Any suggestions for repair? Also, before I bring it into the Apple store i want to remove the fact that's its been jailbroken. How do I do a complete restore of the phone then re-add all my contacts?

2. Restore
3. Set up as new
4. Resync contacts
