Can you someone chime in and help me out please.

Noticed this week that on my iMac, when I go to YT to watch a video, it was looking different.

On my iMac, when I go to watch a video, I get the following:

Youtube logo/home button on the left hand side towards the top where it should be.

No comments show up, nothing on the video in the description under the video.

On the right hand side, it's blank. I have no "Up Next".

I get the YT home button where it should be.
The video I clicked on right smack in the middle.
That's it.
Everything else on the page is white.

Is there a setting or something I clicked on by accident?

It normally happens if the internet connection is bad and youtube is loading for a long time and then it could be, that the appearance is like you mentioned. Reload normally solved this "problem" for me. If its something else than check this out:

Maybe you changed something on the youtube layout. It is also possibe to switch to the old desktop.
