I just noticed this in my system preferences the other day. I have restarted a few times, but it is still happening. What is the problem and how can I fix it?

The icon that is messed up is the Universal access icon. Here is a complete shot of my System Preferences window.

I just tried both the repairing my disk permissions and trashing the plist file, but neither worked.
 
Get rid of the ~/Library/Caches/com.apple.preferencepanes.cache file

(You could just delete everything in the Caches folder in the Library folder that's in your user's home folder.)
 
awesome, thats the same problem i was gonna ask about, except i had it with the speech preferances button. shows that sometimes it does help to search first post later!

EDIT: just so add, it was Mitthrawnuruodo's post on deleting the cache that worked for me, without a restart.
