I changed a few of them on my Mac, I was expecting them to update automatically as it's all iCloud based, however it didn't. I turned off keychain and deleted them, turned it back on... nothing, tried turning off the phone and back on and nothing.... Had it connected to wifi and plugged in but still nothing...

I'm really thinking about just turning this crap off if it means I cannot seamlessly just have my passwords on all devices. I mean is it going to be a big struggle every time I want to change a password?

I'm assuming you are referring to Safari passwords? If so, I don't think passwords work if Safari is in a private tab.
