I've never had to upgrade my phone before so I'm not entirely sure what specific things I should be worried about in terms of upgrading.

I have an iPhone 4S that is with Telus and has a device balance of $300.00 CAD (That is, I can purchase the new iPhone 5 for (retail price with contract) + $300.00).

I also have someone who is willing to purchase my iPhone 4S for $300.00.

So,

(1) SIM Card: What do I do with this? Can I buy a converter and put my current SIM into the iPhone 5? Do I need to talk to Telus about this?

(2) Can I buy the iPhone 5 anywhere? Wal-Mart, Telus directly, Best Buy?

(3) How do I notify Telus about this and change my account to recognize that I have an iPhone 5? Does Bestbuy/Wal-mart do this for me?

(4) My account with Telus is under someone elses name, do I need them to sign anything?

Thanks

Take your sim out, it will be worthless if you activate another sim on your number.

You should be able to buy it at any retailer, I howerver dont' know your carrier's specifics.

When the phone is activated, they will know.

Not sure about your carrier but typically the account holder needs to be present for the transaction to take place.
