How do I make a scanned document/form editable/fillable?

I was wondering if you guys could help me out.

My Clinical Instructor gave us a form that she wants us to complete as part of our care plan. She prefers to have us type our information in, of course these days, not many have typewriters :p So I was wondering how, if possible, can I scan this form in and have it to where I can fill in the required information? I know I have filled in some PDF forms like this before, where the spaces are set to be filled in and fit properly. Any help with this is greatly appreciated, thanks
