How Do I Prepare an iPhone For Sale?

My wife had her iPhone 4 replaced in August because of reception issues. She got the iPhone 4S and is looking to sell her phone. I've never sold and old phone, so I'm not sure how to go about selling it. How do I wipe all of the info off of it? Do I sell it with the sim card?

Any advice would be appreciated.

Full restore. Keep SIM.

You can take the SIM out and go to Settings>General>Reset and then Erase All Content and Settings as well.
