How do I remove Lockinfo

I downloaded Lockinfo and I can't uninstall it. I removed it from cydia and it is still loaded on my lockscreen and it is still in the settings area. Is there a way to remove it? Please help. Thanks.

Well, I have a related issue: I deleted LockInfo but now I don't get any notifications anymore. Missed calls, SMS, ... it never shows up anymore on my lock screen. How do I re-enable this after uninstalling LockInfo?
