How do I unlock this ebook PDF without Adobe Reader?

I got this ebook PDF for a nutrition course and I've attached a screenshot of what it looks like in Preview. When I put in the user name and login, the Submit button does nothing. I would like to avoid putting Adobe Reader that takes over everything it can on the system if at all possible, so has anyone else come across this kind of protection or what other apps can be used to work with it?
