How do people get their app "out there"?

If I spent a long time developing an app and then put it on the app store, how do people find it?

I mean with websites you can make a website and it'll just sit there and people won't know to look for it or find it.

With 1.3 million apps on the store, how the heck do people actually make money with their apps?
