How do you completely restore the phone back to factory?

I'm trying to do a restore on my iPhone 4S...I've pressed the "Restore" in iTunes and it reinstalls the software but when it starts up again, it still remembers my Apple ID and fills it in automatically which tells me it didn't completely wipe everything out.

This happens even before the part where you "Setup as new phone" or "Sync with old phone" in iTunes.

Anyone know?
