I get the "message delivered" receipt on my 4S when I iMessage with another iPhonist, but I've read that there are "read" receipts, too. Where or how do I flick the switch that turns on this function?

This is done by going into your system preferences and choosing the messaging menu.

The second option under messaging will allow you to show the message as read (The default is off). You also have to make sure this is on on both person's iPhones, otherwise it will not work.
 
Just to add to spacesamurai's answer.. it's really out of your control, the other person has to turn this feature on in order for you to see that they read it. I'm sure most people want to keep this off for obvious reasons.
