You don't. The ringtone editor is built into iTunes.

I haven't used Garageband for this so I can't answer the OP's question, but Fission has been working nicely for me.

Yes you can (m4a at least). Open Garageband and drag the file into the window, then edit it down. That's how I make ringtones.
It says it wont work with protected files... any way around this??
Burn it to a CD and import it back in again?
Burning a protected file to a CD and importing never got rid of the protection for me before...
