I've heard several people refer to this... but you cant open an m4a / m4p file in garageband... so how are you doing it?

You don't. The ringtone editor is built into iTunes.

Some people are clipping songs in Garageband and then using something like iToner to upload them to their phones, instead of paying Apple twice.

I haven't used Garageband for this so I can't answer the OP's question, but Fission has been working nicely for me.

Yeah, thats what I mean by using garageband to make ringtones... how do you "clip" a song to make a ringtone??

Yes you can (m4a at least). Open Garageband and drag the file into the window, then edit it down. That's how I make ringtones.
