Did a search and come up empty..

How does the iphone differentiate between sleeping Vs awake? Is it sound? motion? both?

Just wondered.
 
You can download the app and it ask you to turn it on and place it near your head on the bed face down. I suppose that it measures your movement. You can also set a range of wake up times and it suppose to wake you up at the top of your REM cycle. I downloaded it but haven't tried it yet.

It doesn't. It requires a third party app to sync sleep data.

3rd party apps do it just fine. Tell it you're going to sleep and place it next to your pillow. It measures the quality of your sleep by measuring how often you move and how much you move. It's pretty informative.
 
I know there are some apps that you leave on your bed while you sleep and they measure your activity during the night. I think those apps will share your data with Health kit.
