Exactly as the title states- How does it work?

Like, if I report a post, does a moderator that is online at the time get a pop-up sort of like a new Private Message saying that I reported this post?

A new thread is created in a mod only area that provides a link to the reported post and the reason that you supplied when you reported it. Emails and carrier pigeons are dispatched to all the mods. Some of us monitor post reports by watching the forum spy others monitor them with email or rss feeds. Some of us have software set up to provide a pop up on our computer. The thread allows us to discuss the issue if it is something that requires discussion. Reported post threads are closed when someone resolves the issue so that we can tell at a glance what issues are pending and how long they have waited.

I have an advanced notification system should I be out of cell tower range.
