Last year I remember they had 2 fans in 13" inch model with Touch Bar. This year the two USB port model has only one fan according to fixit teardown. Can't find any teardown with 4 USB ports.

I can't really grasp my head around why there are two different internal designs. What are the consequences of it. Does the 2 fan model behave much better under load?
 
The 4 port version remains the same as the "touch bar" version since 2016, it has always had 2 fans. The main reason is to adequately cool the 28W CPU used inside, as opposed to the 15W on the nTB / 2 port version.

It is difficult to compare the two in cooling efficiency since the two TDP wattage CPUs are fundamentally different thermally. The current 15W used in the 2019 2-port version is already quad core, while having a low base clock to begin, but a high turbo clock. This in theory (and in practice as tests have shown) is a more efficient use of the thermal potential of this MBP design as long as you are pushing the maximum load continuously, which probably describes most use cases. The 4-port 2-fan 28W version on the other hand stays at a higher starting point in terms of clock but not really going much further than the 15W version if at all. It probably can maintain maximum load for longer periods but I have yet to see real tests to confirm so.
