How much does a cracked lens on a 12 Pro Max cost to get fixed with AppleCare+?

If it's still $99 at the store I might as well just do an express replacement.

They'll most likely replace the phone as the camera lenses are part of the rear glass assembly and are anchored from inside the device.. I heard they now have an entire rear glass frame assembly and only have to swap a few parts from your phone to the new assembly but not sure if that's what they'll do for your particular issue
