How to create an icon in OS X?

Well see I tried pic2icon but when I would go to use the .icns file it created it wouldn't work. When I would paste it to the programs icon I wanted to change it would change it to the Preview file icon, the program used to open the file. When I would use the Icon Composer that comes with OS X it would do the same thing as well. Then I gave img2icns a try and I don't know how but it worked for me...
But then when I went and tried to use one of the .icns files created with img2icns on my friends MacBook, it was doing the same thing on her computer that it was doing on mine (by changing them to the Preview file icon).
