How to create poll?

When I create a new post, how do I add a poll ? I searched the forums, user guides and FAQ's and couldn't find any instructions. When creating a new thread, there does not seem to be any button to create a poll. There is a checkbox that says "Yes, post a poll with this thread" - but it doesn't indicate how to create that poll.
