Ripping copyright protected DVDs may be illegal in your country. Please consult all local laws before proceeding.
Note: I've searched the forums high and low and haven't seen a simple How To on ripping DVD's specifically for a iPhone/iPod Touch and the Mac Guide on ripping video to iPod Video's just ain't going to cut it. So instead of making a Mac Guide for this, since no one seems to know they exist, I figured I would post a thread explaining it ... with pictures! so anyone can understand and hopefully curb the number of threads about it.
Software needed:
HandBrake
0. Quit DVD Player if it automatically starts after inserting DVD
1. Select Folder:
2. Click Toggle Presets in upper right hand corner:
3. Choose iPhone / iPod Touch preset:
4. Click 2-pass encoding & Turbo first pass:
*This will take twice as long but the resulting file will look better (commercial grade) with no file size change.
5. Click Start to begin.
The encoding will take about 2 hours for a 1 hour 30 minute DVD using a 2.16GHz MacBook.
Importing DVD Artwork into iTunes
a. Once complete, drag & drop the file into iTunes to import.
b. While importing go to Amazon.com to grab artwork (search for movie, then click on picture for full size and drag to desktop).
c. Once imported into iTunes select the movie.
d. In the Menubar click File then Get Info
e. Click the Artwork tab
f. Go to desktop
g. Drag & drop DVD artwork downloaded from Amazon into Artwork window.
You now have a high quality iPhone/iPod touch version complete with chapter markers and DVD artwork. All that's left to do is sync.

Can you guess what movie it is?
Footnote:
If you find that you get an unusable file or when playing the audio/video is missing you can try ripping with MacTheRipper. Just read the help file that comes with it and you'll have no problem.
