How To "Put Together a Poll

Hopefully this is the pertinent place for this inquiry. I did try and search the site for guidance, but could not find anything.

I want to put together a poll about backup software. Basically the poll would look like the following:

" Which software do you use for backups?

- Time Machine
- SuperDuper!
- Carbon Copy Cloner
- Other
- Do not perform backups"

Would appreciate assistance. Or if someone actually wants to construct and post such a poll, that would be fine too.
