I deleted some music/podcast in the past - in the wrong way i guess - but now it comes up when im creating smartlists etc. as an entry but when i click to play it is unplayable & comes up with 'attention' sign. Then i have to excluded it out of the list.

Is there a sure fire way to completely refresh this 'background' list that itunes keep so it reflects my library as it now?

You can delete items in Smart Playlists. You have to use the shift or option key (I forget which) when you press delete.

(select items within smartlists > press option key & delete)
