I've always had my activations done in the store. Will itunes activate my phone when I connect and restore from my iPhone 4's last backup or do I need to contact ATT? Do I have to put the SIM card in the phone? Sorry if these are silly questions.

Don't think you need to plug it into anything to activate with iOS 5. Just turn it on and use it.
 
I ordered from AT&T directly and they sent me an email with a link, saying to open the link and enter my order code or something when I receive the phone, but not before then.

If you ordered from Apple, I would imagine the phone will either walk you through activation when you turn it on or you'll activate in iTunes.

Apple is really making a push to have iOS devices not be dependent on a computer, though, so I would imagine there is some sort of computer-less solution.
 
The sim card will already be in the phone. It just activates the new one and deactivates the old one.
 
Sweet. This link I went to from att wanted the last four of my social and a verification number.
